1. Choose a plan that fits your needs and budget:
- Healthcore Access
- Healthcore Advantage
- Healthcore Essential
- Healthcore Enhanced
- Healthcore Encompass
2. Complete the application
3. Mail in the application with supporting documents:
Proof of NYS RESIDENCE (may submit any of the following)
- NYS driver license
- Utility bill (gas, electric, cable, etc.) or postmarked mail with address
- Letter/lease/rent receipt from landlord
- Property tax records or mortgage statement
Proof of EMPLOYMENT (may submit any of the following)
- Pay stubs
- Letter from employer
- Documentation sufficient to demonstrate self-employment
Proof of HOUSEHOLD INCOME (May submit any of the following)
- Pay stubs
- Award letters/ benefit checks
- Business records
- Letter from employer
*Healthcore application and required documentation must be received by Benefit Specialists of NY by the 20th of the month for enrollment the 1st of the next month.
